Cyrtosperma johnstonii carries large hastate to arrow-shaped leaves above long, thick petioles. Young plants can show vivid pink along the main veins, while older blades become predominantly green and may develop reddish-brown colouring beneath. White, green, brown and pink mottling adds further detail to the petioles. Juvenile foliage holds the strongest pink vein colour, which commonly fades as the blades broaden and mature.
At the same time, the petioles become taller, heavier and heavily armed, with prickles merging into characteristic oblique combs. Basal shoots eventually produce a clump of large crowns, and an established specimen can reach about three metres high.
