Cereus forbesii produces upright blue-green columns divided by four to seven broad ribs. Small woolly areoles sit along their edges and carry stout yellow-brown spines, usually with a longer darker central spine. The waxy blue coating is strongest on young stems and emphasizes the clean vertical lines between the armed ribs.
The columns thicken and shift towards grey-green or olive as they age. Branches gradually develop around the lower structure, turning a solitary young cactus into a shrubby or tree-like group that commonly reaches about 2 m and can grow considerably larger outdoors. Mature plants may produce large funnel-shaped flowers, white to pinkish inside and reddish outside. These open from the sides of the columns and may be followed by fleshy fruit, placing broad pale forms against the vertical ribs.
